Mercer County COLT (Community Organizations Linking Together) Health 

is a group of volunteers and employees of community organizations and agencies who are committed to improving the health of the people of Mercer County. With support from Mercer County Health District and Mercer Health, the members of COLT Health act as the steering committee for Live Well Mercer County.



The Mission of COLT Health is Live Well Mercer County

The components of a thriving community include many things.  Certainly without thriving residents, the other components of a community will weaken as well.  In order for all residents to live well, the health, safety and wellness must be a priority.  The mission of COLT Health is to influence and improve the culture of well-being for all of Mercer County.

Needs Assessment and Improvement Plan

What is a Community Health Needs Assessment (CHNA)?

A CHNA is an examination of the health status of a community used to identify areas of concern.

Mercer County’s most recent CHNA was completed in 2019. The full assessment can be found in Mercer County’s 2022 Community Health Report

What is a Community Health Improvement Plan (CHIP)?

A CHIP addresses public health concerns based on the results of a community health needs assessment. The plan includes benchmarks and improvement goals focused on improving the overall health status of the community.

Mercer County’s 2022-2024 CHIP addresses concerns identified by members of our community via the 2021 Community Health Needs Assessment. The full plan, which includes strategies and action steps, can be found in Mercer County’s 2022 Community Health Report.  Below are the four targeted health improvement priorities.
